HOW TO ORDER

1.    Using the enclosed order form, list each publication you want to order and provide the ISBN and price.

2.    Calculate the total price.

3.    Determine the appropriate postage and handling charge using the following schedule:

       Within Canada:
       First item to Ontario, Quebec: $5.95
       First item to Manitoba, Saskatchewan, Nova Scotia, New Brunswick, Prince Edward Island: $7.95
       First item to Alberta, British Columbia, Newfoundland and Labrador, Yukon, Northwest Territories, Nunavut: $8.95
       Each additional item: $1.75

       Outside Canada:
       First item to any destination: $6.50
       Each additional item: $2.25

       Note: Postage and handling charges reflect the current postal rates set by Canada Post and match the rates set by the provincial office of OGS.

4.    Include your name, complete address and postal code or Zip code.

5.    Make your cheque or money order payable to "Kingston Branch OGS". We must receive full payment before we can process your order.

6.    Prices are in Canadian dollars. If you are ordering from the United States, we require payment of the full Canadian amount by cheque or money order in US dollars.

7.    If you are ordering from outside North America, we require payment of the full Canadian amount by money order or bank draft in US dollars.
